A Note from the Executive Director

    Hello Friends and Families,

We have started having residents fill out a screening questionnaire once they return from leaving the community with the following questions:

    1. Were you away from the property for longer than 24 hours?
    2. Do you have a cough, sore throat, fever or shortness of breath today?
    3. Have you been in close contact with someone with known COVID-19 or flu-like symptoms while away?
    4. Did you use proper hand hygiene, use your mask and practice social distancing while around others?
    5. Do you agree to self-monitor for signs and symptoms for 14 days upon return and inform staff (RSC, Nursing or Executive Director) if you develop symptoms?

Please help us remain safe by ensuring that masks are being worn, social distancing is followed, and proper hand sanitation is being done. Refrain from contact with anyone with signs or symptoms of COVID-19. If you come in contact with someone with signs or symptoms of COVID-19, let your Health Services Director know.
